We are currently accepting applications for our EFSP program. We are able to assist with: ● ONE MONTH of utility assistance OR● ONE MONTH of rental assistance OR● ONE MONTH of mortgage assistance We are UNABLE to assist with Security Deposits for rentals or utilities. To apply, please email us at gro.orezeveihca%40ofni with the following information: Rental Assistance: ● Name and DOB of ALL household members● Copy of ID for ALL Household members● Social Security number for all adults● Copy of Lease Agreement ● Letter of Need stating WHY you are in need of assistance● At least TWO (2) months of pay stubs● Proof of Benefits (SNAP, TANF, SSI, GA, etc.)● Ledger from Landlord stating the AMOUNT DUE and for which MONTH● Contact phone number for head of household● Email address if available Utility Assistance (Electric, Gas, Sewer, Water): ● Name and DOB of ALL household members ● Copy of ID for ALL household members ● Social Security number for all adults● Letter of Need staying WHY you are in need of assistance● At least TWO (2) months of pay stubs ● Proof of benefits (SNAP, TANF, SSI, GA, etc.● Utility bill OR Cut-off/shut-off notice● Metered utility verification form OR Billing and payment history from vendor ● Contact phone number for head of household ● Email address if available
The following forms must be completed, signed, dated, and returned to Achieve Zero before your request for assistance can be processed.
